Learn what MedWise Seniors collects, what it does not collect, and how printable tools and anonymous evaluation features work.
The most important point
Do not enter or send personal medical information. Printable forms are designed to be completed on your device or on paper. MedWise Seniors does not transmit or store the medication, diagnosis, allergy, provider, or patient information typed into those forms.
Information used for website measurement
- A one-way visitor identifier used to estimate unique visitors
- The page path, visit time, and referring domain
- Anonymous optional evaluation answers and device codes
- Aggregate presentation, partner, attendance, and distribution counts entered by the project director
Information not intentionally retained
- Raw IP addresses, names, email addresses, birth dates, or account credentials
- Medication lists, diagnoses, allergies, schedules, or provider information
- Information typed into printable caregiver or patient forms
Email links open the visitor’s own email application. Information sent by email is handled by the visitor’s email provider and the recipient’s email service, not by the website form.
