What is measured

  • Self-identified older adults and caregivers participating in an optional anonymous check-in
  • County or Greater Birmingham service area
  • Education guides completed and large-print resources opened or printed
  • Pre/post medication-safety knowledge and confidence
  • Intentions to update a medicine list or speak with a pharmacist
  • Optional 30-day actions completed
  • Community presentations, attendance, partner organizations, referral sources, and printed-material distribution

Reporting rules

Website visitors are reported separately from self-identified participants. A page view is not counted as a senior served, a health service delivered, or an improved outcome. Targets are always labeled as targets rather than results.

Privacy safeguards

The evaluation does not request or store names, email addresses, dates of birth, diagnoses, medication names, allergies, schedules, provider information, or entries typed into caregiver and office forms. An anonymous device code may connect voluntary starting, follow-up, and 30-day responses.
